INSTRUCTIONS
There are several ways to use these pdf files:

1. Save the files and complete them electronically or manually.

• Right-click on one of the links and select "Save Link Target As...” to save the file onto your PC or network.
• Once the files are saved to your system, you may open and fill them out online, then print them out OR print out blanks and fill them out manually.
• Adobe Reader is required to read these documents, available for free at http://www.adobe.com/products/acrobat/readermain.html. (Older copies of Adobe Acrobat also work.) Note: data on forms filled out in this manner cannot be saved electronically.

2. Open one of the links directly in your browser, and proceed as before, again using Adobe Reader.

3. If you want to save completed forms in electronic format, you need additional software, which must be purchased from Adobe. The pdf Distiller application allows you to save each completed form separately.

No matter which method you use, a signed original form must be returned to the UCSF Foundation. Download the "Instructions for Completion..." form above for further information.
